How do you get the codex?

So, I've read online that you obtain the Codex after your 3rd escape attempt by talking to Achilles and he gives it to you. The problem is, I'm around attempt 30 and have even beaten the final boss and still have yet to obtain it. I've spoken to Achilles multiple times throughout the entire game as well.

Is the codex unbiased or objective?

The Codex functions as a semi-magical record of Achilles' own travels and experience, both on the surface (e.g. with the Olympian Gods) and in the Underworld; therefore, it is not unbiased or objective, but is written from Achilles' point of view, and references his personal opinions, relationships, history, and understanding.

Where can I find a codex of the underworld?

If you haven’t done so already, make sure you visit Achilles in the House of Hades who will gift you the Codex of the Underworld early on. This keeps track of enemies, family members and importantly, who you have gifted Nectar to. Make sure to check in with characters dotted around the House of Hades.
